Please print this form and send it fully signed to our fax number on Hawaii (001-808-985-7501) or to our German Office at +49 (0)5031-178092. We will request the urn with the ash of the deceased from the crematorium upon receipt of the order.

Alternatively you can of course ship the urn to our address yourself. In this case please read the shipping instructions before you send the ashes.
